Patient safety policy launched

The National Patient Safety Agency has officially launched a “Never Events” policy which outlines serious patient safety incidents that should not occur.

PCTs will use this to help focus commissioning of the safest services for their patients. They will be working with healthcare providers on using the policy locally, and incidents will be monitored and publicly reported on an annual basis. Eight core Never Events have been identified, including: wrong site surgery, inpatient suicide using non-collapsible rails and wrong route administration of chemotherapy.
