New local patient safety incident data

NHS staff are more likely than ever to raise a patient safety concern as new data, released by the National Patient Safety Agency (NPSA), demonstrates an upward trend in safety reporting.

Last month, the NPSA began publishing Organisation Patient Safety Incident Reports from each NHS Trust or local health board in England and Wales. The latest data show that over 65% of all patient safety incidents result in no harm to the patient. The most common problems are patient accidents, followed by treatments and procedures and medication related incidents.
